IRMAA brackets 2026

IRMAA, the income-related monthly adjustment amount, is an extra charge on Medicare Part B and Part D premiums for people whose income is above set thresholds. It is based on your tax return from two years earlier, so 2026 IRMAA uses 2024 income. The brackets below come from the official 2026 figures.

What is this dataset?

It is the 2026 table of Medicare income tiers and the monthly amounts added at each tier: the standard Part B premium, the Part B adjustment, and the Part D adjustment, for individual filers, joint filers and married people filing separately. The figures come from the Centers for Medicare and Medicaid Services announcement and Social Security's published table.

How do I read the table?

  1. 01

    Find your filing status

Individual, joint or married filing separately.

  2. 02

Find your income from two years ago

Use modified adjusted gross income from that year's return.

  3. 03

    Read your row

The Part B column is your total monthly Part B premium; the Part D column is added to your drug plan's premium.

What rule sets IRMAA?

Federal law requires higher-income Medicare beneficiaries to pay a larger share of Part B and Part D costs. Social Security decides whether you owe IRMAA from the tax data the IRS sends it, and CMS sets the dollar amounts each year. Income tiers are adjusted for inflation. If your income has dropped since the lookback year because of a life-changing event, including retirement, you can ask for a new determination.

Where to go next

IRMAA is one input in the FEHB and Part B decision at 65. Compare both paths in the FEHB vs Medicare tool, and read FEHB and Medicare at 65.

The year you retire can change your income picture for IRMAA.

Questions about this data

What income is used for IRMAA?

Social Security uses your modified adjusted gross income from your federal tax return two years before the premium year: adjusted gross income plus tax-exempt interest. For 2026 premiums, that is generally your 2024 return.

Can I appeal IRMAA?

Yes. If your income dropped because of a life-changing event such as retirement, marriage, divorce or the death of a spouse, you can ask Social Security to use a more recent year with form SSA-44. Retiring from federal service is one of the qualifying events.

When are the 2027 IRMAA brackets published?

CMS usually announces the next year's Part B premium and IRMAA amounts in the fall. This page switches to 2027 when the figures are published, and keeps the date beside the table.

Does IRMAA affect FEHB premiums?

No. IRMAA is added only to Medicare Part B and Part D premiums. It matters to federal retirees deciding whether to add Part B to FEHB at 65.
